·         Health Care Program for Children with Special Needs - Health Care Program for Children with Special Needs (HCP) is a resource for families, health care providers and communities. It is a federal and state funded medical assistance program.  It is the goal of HCP to improve the health, development and well being of Colorado’s children with special health care needs.  HCP serves children from birth to 21 years of age who have or are at risk for serious physical, behavioral or emotional conditions.

·         Early & Periodic Screening, Diagnosis & Treatment (EPSDT) - Early and Periodic Screening, Diagnosis and Treatment Service is Medicaid’s comprehensive and preventive child health program for individuals age 20 and under.  EPSDT emphasizes preventative care and complete early treatments.  The mission of Colorado’s EPSDT Outreach and Case management program is to guide families with children eligible for Medicaid to value, access and use appropriate health care services that lead to their health and well being.
